2018-2019 PHYSICAL, EYE EXAM, AND IMMUNIZATION REQUIREMENTS

IMPORTANT REMINDER for all preschoolers (immunizations required for ALL preschoolers), kindergarteners, 7th graders, and any student new to the district.

***BEFORE THE FIRST DAY OF SCHOOL***

PHYSICAL AND EYE EXAM- Nebraska State law requires that you have a complete physical and eye exam done by a physician, physician’s assistant or nurse practitioner. Any school physical form will be accepted. You can find a school physical form on the Crete Public Schools website or at this link

IMMUNIZATION REQUIREMENTS- Bring your completed immunization record to the school prior to the first day of school. You will not be permitted to attend school without proof of immunizations. You can find the immunization requirements in English and Spanish are in the links below.

If your child has not received all of the required immunizations, they can be provisionally enrolled as long as they have begun the immunization series prior to enrollment and continue to receive the shots as per the catch up scheduled. Provide the school with the student’s next scheduled appointment date and time.
